Charge

ASTRO and AAPM are engaging a program to create a Patient Safety Organization (PSO) to provide a nation-wide error reporting process and database. The PSO has a governing group consisting of three ASTRO and three AAPM members. The AAPM considers this effort to be of extreme value to radiation therapy clinics and medical physicists and has therefore decided to form a working group that is taked with monitoring the PSO effort, providing guidance regarding the AAPM perspective, and provide updates and feedback to the AAPM Membership. The current initial tasks of the working group are: 1. Aid the AAPM to develop the Memorandum of Understanding with ASTRO. 2. Pilot the database and provide feedback to the joint working group. 3. Evaluate and monitor how well the event reporting is going. 4. Assess the level of access to important anonymized event data. 5. Report to SC (and PC via its liaison) and provide feedback to the joint working group and the PSO.
