Purpose: We develop and evaluate left breast cancer for intensity- modulated radiotherapy treatment planning₍IMRT₎ by using a knowledge based planning₍KBP₎ model₍Varian Eclipse13.5₎,in order to investigate the feasibility of KBP and evaluate the quality differences of radiotherapy plans.

Methods: A KBP model was constructed from 49 left breast cancer IMRT radiotherapy plans then applied to 30 previous patient plans.30 New patient plans were generated according to the KBP model predicts DVH bands for Target and each organ at risk which were compared to the previously planned DVHs for that patient.

Results: With the KBP, All IMRTauto plans were recognized as clinically acceptable and 54% as clinically improved; 33% of IMRTauto plans were marked equal and 13% inferior to manually generated IMRTman plans when reviewed in a single-blind study by one experienced radiation oncologist. The monitor units of IMRTauto plans were statistically significantly lower than IMRTman plans.

Conclusion: The KBP model is robust and dramatically efficient in clinical application and provides improved planning quality.
