Evaluation of An Automated Treatment Planning Verification System for Intensity- Modulated Radiotherapy Treatment Planning

Purpose: Mobius3D is a verification system for treatment plan dose calculation and treatment delivery. The purpose of this project is to verfiy M3D calculation of planned and delivered dose for IMRT, in order to demonstrate this system can be replace measurement based patient specific QA.

Methods: IMRT treatment plans(NPC、Lung、Breast) were generated and delivered for the head and neck , lung phantoms containing ionization chamber(0.6cc)and film to measure dose. The treatment plans were exported from the TPS to M3D and the dose was recomputed. The measured ionization chamber and film dose were compared to the dose calculated by the TPS and M3D. Next, 30 IMRT treatment plans (10 NPC, 10 Lung, 10 Breast) were selected for actual patients at had been treated and whose plans passed measurement-based IMRT QA. The treatment plans and log files were exported from the TPS to M3D. Then, the planned dose (based on beam information from the TPS) and delivered dose (based on treatment log files) were calculated. The recalculated M3D dose was compared to the TPS. The agreement of both M3D planned and delivered dose to the TPS dose was evaluated using 3D gamma analysis with 3% dose difference and 3mm distance to agreement passing criteria.

Results: The M3D planned and delivered dose distributions calculated for phantoms agreed with the TPS dose, with pass rates of 99% or greater for 3D gamma analysis. In addition, the M3D dose for 30 patient treatment plans were found to agree with the TPS dose, with an average 3D gamma passing rate of NPC、Lung、Breast were 93.32%±1.89%、96.53%±1.40%、95.70%±2.02% for M3D planned dose .The 3D gamma was above 90% for M3D delivered dose.

Conclusion: The results of this study show that the accuracy of M3D planned and delivered dose calculation. M3D has the potential to be replace patient specific pretreatment QA .
