Cortical Gray Matter Volume Changes in Essential Tremor Subgroups

Purpose: To assess the extent of gray matter (GM) volume loss in subjects diagnosed with Essential Tremor (ET) using automated segmentation of MRI T1-weighted images in symptom stratified groups.

Methods: Magnetic Resonance Imaging (MRI) was acquired on 40 ET patients and 25 control subjects. Automated GM segmentation and a voxel-wise comparison of volume was performed with the Statistical Parametric Mapping (SPM) software. Stratification of the data was performed to generate the following group comparisons: all ET vs. Controls, ET with head tremor (HT) vs. Controls, and ET with HT vs. ET without HT. Age, sex, intracranial volume, and Montreal Cognitive Assessment (MoCA) score were regressed out from each group comparison. Multiple comparison correction was performed using extent cluster correction which has been validated and widely used in fMRI analysis.

Results: ET with HT showed a statistically significant decrease in GM volume compared to ET without HT in the left hippocampus (p=0.001), bilateral striatum (p=0.02), and right thalamus/striatum (p=0.03). The following groups showed no statistically significant difference in GM volume: all ET vs. Controls and ET with HT vs. Controls.

Conclusion: Decreased GM volume in ET patients with head tremor in the hippocampus and basal ganglia is consistent with GM volume loss reported in multiple studies on other neurodegenerative disorders.
