An Investigation in Implementation of AAPM Task Group 119 Test Suite Benchmark Plans for Proton Pencil Beam Treatment Planning

Purpose: This study was to implement AAPM Task Group 119 test suite benchmark plans for Proton Pencil Beam Scanning (PBS) delivery system and to evaluate proton treatment planning results by comparison using all benchmark plans criteria and associated clinical goals.

Methods: According to AAPM TG-119 in commissioning, tests and instructions for photon planning, measurement and analysis, a set of test clinical cases have been identified in order to score the accuracy of photon IMRT planning and delivery system. The process also has been expanded into the possibility in VMAT domain. Among these benchmark test cases, we implemented in Proton PBS realm. All contouring, prescription, dose optimization and calculations were performed in Ray Station treatment planning system. In comparison with original clinical goal of photon planning, VMAT and IMRT plans were initiated and planned accordingly with the identical dose prescription and optimized to meet criteria set by TG-119.

Results: All PBS/VMAT/IMRT plans were compared based upon the treatment goal of TG-119 in Dose Volume Histogram (DVH) in referenced dose versus ROI volume coverage/avoidance requirements. All cases in test suite such as prostate, head-and-neck, C-shape and multiple targets with its own prescription dose were tabulated respectively. The PBS acceptable level of clinical goal was set to be more restricted than photon beams if feasible..

Conclusion: It has been recognized that TG-119 test case suite is useful to compare different optimization modules in photon delivery. Proton PBS planning was challenging and the existed complexity needs to be tested just like photon IMRT or VMAT as part of quality assurance before dose delivered. This investigation proved that TG-119 is a resort to provide crucial baseline in Proton PBS planning process and can be expended with other proton center with institutional effort as the initial stated in TG-119..
