  1. Present key and significant medical incidents reported since the original report “High dose-rate brachytherapy treatment delivery: Report of the AAPM Radiation Therapy Committee Task Group No. 59”
  2. Examine potential risk, incidence, and severity specific to HDR brachytherapy utilizing FMEA as outlined in TG100.
  3. Revisit the checklists in the original TG59 report and update these with model QA and QC forms, HDR plan check guidance, and dose accumulation (EBRT + HDR) recommendations in light of deformable registration.
  4. Reexamine the utilization of modern technologies and safeguards that have been introduced since the original report, which have been designed to reduce and/or automate QA and QC processes, along with emerging QA-oriented technologies.
  5. Update and regenerate the model procedure workflows for different scenarios of clinical practice.
